Health requirements for Lab Schools students:
Physical exam:
New students at any grade level are required to submit, before the beginning of the school year, a physical exam administered within a year of entry and a complete record of immunizations. For specific requirements, click on your child’s grade level below.
Returning students in kindergarten, sixth and ninth grades are required to submit,
by the beginning of the school year, a physical exam administered within the past year and a record of any required immunizations. Any needed immunizations will be requested individually by mail. For specific requirements, click on your child’s grade level below.
Children returning to the second year of nursery school are not required to submit exams if they did so the previous year.
Dental exam:
Students entering kindergarten, second and sixth grades are required to submit dental
exam forms by April 30 of the school year. Forms should be directed to the Nurse’s Office in Blaine 107.
Eye exam:
Students entering kindergarten or enrolling in a school in Illinois for the first time are required to submit an eye exam by the beginning of the school year. Exams must be administered by a licensed optometrist, ophthalmologist or a physician qualified to do eye exams. It must be completed within the year prior to kindergarten or other grade entry. Forms should be directed to the Nurse’s Office in Blaine 107.
